Do I need a doctor's referral or prescription?
In most circumstances, it is preferred that you come with an order from your primary care physician (PCP) or other specialist that is able to refer you. Some insurances require a referral from your PCP before they will authorize any occupational therapy treatment. Check with your insurance company to see if a referral is required.  Please note: Therapists are not able to diagnose or fill out certain medical forms. Click HERE to download one of our referral forms that you can bring to your next doctor's appointment.

What types of payments do you accept?
At our office, we accept cash, checks, and credit cards (Visa, MasterCard, Discover, Google Pay and Apple Pay. We do not accept American Express). You can make a payment online, over the phone by calling us at (503) 692-5210, or by mailing in a check.
