Do I need a doctor's referral or prescription?
In most circumstances, it is preferred that you come with an order from your primary care physician (PCP) or other specialist that is able to refer you. Some insurances require a referral from your PCP before they will authorize any occupational therapy treatment. Check with your insurance company to see if a referral is required.  Please note: Therapists are not able to diagnose or fill out certain medical forms. What types of payments do you accept?
At our office, we accept cash, checks, and credit cards (Visa, MasterCard, Discover, Google Pay and Apple Pay. We do not accept American Express). You can make a payment online, over the phone by calling us at (503) 692-5210, or by mailing in a check.

Should I bring anything to my visit?
For you first appointment, make sure you bring your insurance card, ID, and doctor's referral/order. If it's a worker's compensation injury, please bring any information regarding your claim including your claim# and adjuster's contact information. You can complete the new patient forms on the website, or you can come in 10 minutes before your scheduled appointment to complete the paperwork. There may be additional forms that may be given depending on your insurance's prior authorization requirements.
